If you wish to retrieve your histology samples, such as paraffin blocks, microscopic slides or wet samples (if remaining tissue is available), AHLab has a secure and structured process to facilitate your request. By completing the Authorisation and Release of Histology Sample form , you provide the necessary details and confirmations for us to process and release your materials safely.
Step-by-Step Process:
1) Notify the laboratory of material request
By emailing [email protected]. Detail your full name, date of birth and the material being requested. This will allow us to collate the required material and have it ready for you. A minimum of one week notice must be given for any material request.
2) Complete Personal Information
Begin by filling in your name, date of birth, address, and contact number in the form’s personal information section. This information helps us verify your identity.
3) Specify Requested Items
In the section labelled “Information to be Used or Disclosed,” select all materials you need, such as pathology reports, tissue remains, paraffin blocks, or microscopic slides. Check all applicable items to clarify your request.
4) Declaration
Confirm in the declaration section that you understand the infection control risks (as the tissue is not sterilised) and that you are responsible for the material upon release. Indicate that no other parties, such as joint custodians, oppose the request.
5) Identity Verification
Bring with you a photo ID for verification. If you are a representative (not the patient), specify your relationship to the patient, provide evidence of authorisation to collect the material, bring a photo ID for verification for yourself and a copy of the patient’s.
6) Witness Signature
The staff handling your request will be acting as your witness and will provide the following details on the form signature and date, confirming the declaration’s validity. The staff member will also go through the declaration section with you to ensure that this section is fully understood and answer any questions.
7) Submit the Form
Once completed, submit the form to AHLab for processing. Retain a copy for your records.
By following these steps, you can confidently and securely request your histology materials from AHLab.
Please note that AH Lab is not responsible for the tissue’s use after release. If a funeral director will handle the tissue, they must verify the patient’s identity.
